assurer le contenu, la gestion et le suivi d'un dispositif de prise en charge éducative de patients diabétiques de type 2 ou présentant un syndrome métabolique, dans le cadre des actions prévues notamment aux articles D.6321-1 à D.6321-7 du code de la santé publique et L.162645 du code de la sécurité sociale; Ce dispositif vise notamment à un meilleur accès à l'éducation thérapeutique, notamment sur la prise en charge diététique et sur l'activité physique, une amélioration de l'équilibre du diabète et de la prévention des complications, un développement des actions pour la prise en charge des pré-diabètiques, une optimisation de la qualité des soins en développant des actions de formation des professionnels de santé, une amélioration de la coordination des soins autour du patient détenteur d'un dossier partagé
